Insurance Information

We are pleased to file your insurance for you. As a courtesy we will file with both a primary and secondary insurance. However, it is important to remember many insurance companies have many stipulations that may affect coverage. It is ultimately your responsibility to understand your insurance benefits and limitations. Please remember you are fully responsible for all fees charged by this office, regardless or your insurance coverage.

Please check with your insurance provider to be sure we are IN NETWORK with your plan.

If we are out of network you may still come to our office. It may or may not affect your insurance coverage. Please check your benefits package or with your insurance provider.

We offer an inoffice Discount Plan for patients without insurance, please check it out under the Discount plan tab.
